On or about January 12th 2017, at the above addressed jobsite located at 3400 W 45th St. West Palm Beach, FL 33407, the employer did not implement an effective hearing conservation program for employees who were exposed to noise levels of 92.5% dose or an equivalent sound level of 89.4 dBA. The sampling was performed for 488 minutes and zero exposure was assumed for the un-sampled time of 120 minutes.  b. On or about January 12th 2017, at the above addressed jobsite located at 3400 W 45th St. West Palm Beach, FL 33407, the employer did not implement an effective hearing conservation program for employees who were exposed to noise levels of 83,43% dose or an equivalent sound level of 88.6 dBA. The sampling was performed for 493 minutes and zero exposure was assumed for the un-sampled time of 60 minutes.
