29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):            a) On or about January 17, 2017, employees were exposed to the hazard of falling approximately two stories (20 feet) from second story top plates while performing carpentry work without fall protection.     b) On or about January 17, 2017, employees were exposed to the hazard of falling approximately 12 feet from a second story window/wall opening while performing carpentry work without fall protection.  c) On or about January 17, 2017, employees were exposed to the hazard of falling approximately 10 feet from a second story deck while performing carpentry work without fall protection.   d) On or about January 17, 2017, employees were exposed to the hazard of falling approximately 10 feet from a hole in the first story deck while performing carpentry work without fall protection.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection:         a) On or about January 17, 2017, employees were provided with electrical tools and equipment connected by cord and plug to a Honda 6500 watt portable generator without the use of ground fault circuit interrupters or an assured equipment grounding conductor program.           No abatement certification or documentation required for this violation.
