(a) Location - 94 Essex Street, Saugus, Massachusetts: On or about, January 17, 2017, employees were not protected from falling while working at a heights of 9 feet 4 and half inches above a lower level. Clemens and Sons Construction and Roofing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 960091, citation number 01, item number 1/A and was affirmed as a final order on 3/26/2014, with respect to a workplace located at 155 Lawton Avenue, Lynn, Massachusetts.
