29 CFR 1910.178(l)(4)(i): Refresher training, including an evaluation of the effectiveness of that training, was not conducted as required by paragraph (l)(4)(ii) to ensure that the operator had the knowledge and skills needed to operate the powered industrial truck safely: a) Atlas Logistics at 10251 E. 51st Ave, Unit A, Denver, CO 80239: On and before January 11th, 2017, the employer did not ensure refresher training, including an evaluation of the refresher training, was conducted after employees had been involved in accidents. Accidents such as, but not limited to, those occurring on December 9, 2016, and July 12, 2016, involved employee injuries, and the employer did not conduct refresher training. This condition exposed employees to struck-by hazards.
