29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    a. JMS Inc.: On or about January 18, 2017, and at times prior, employees were exposed to respiratory hazards without the employer having implemented an effective respiratory protection program in place while employees were performing operations including, but not limited to, machining, welding, shell casting, wax molding, metal casting and sand blasting operations.     b. JMS Inc.: On or about January 18, 2017, and at times prior, employees were exposed to respiratory hazards without having given employees medical evaluations prior to being required to wear respirators while employees were performing operations including, but not limited to, machining, welding, shell casting, wax molding, metal casting and sand blasting operations.    c. JMS Inc.: On or about January 18, 2017, and at times prior, employees were exposed to respiratory hazards without having fit testing employees wearing tight fitting respirators while employees were performing operations including, but not limited to, machining, welding, shell casting, wax molding, metal casting and sand blasting operations.    d. JMS Inc.: On or about January 18, 2017, and at times prior, employees were exposed to respiratory hazards without having thoroughly trained employees on the potential respiratory hazards that could be encountered during daily operation while employees were performing operations including, but not limited to, machining, welding, shell casting, wax molding, metal casting and sand blasting operations.    e. JMS Inc.: On or about January 18, 2017, and at times prior, employees were exposed to respiratory hazards without having regularly evaluated the effectiveness of the respiratory protection program.
