On or about January 18, 2017, at the above addressed jobsite, employees performing carpentry operations on a roof fifteen (15') feet six (6") inches above the ground were not protected from falls, thereby exposing employees to fall hazards.    b. On or about January 18, 2017, at the above addressed jobsite, an employees performing residential carpentry operations next to window openings on the ground floor, nine feet (9) five (5) inches above a basement access, were not protected from falling, thereby exposing the employee to fall hazards.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:     a.  On or about January 18, 2017, at the above addressed jobsite, employees were not provided with safe means of access to the top of a house that was fifteen feet (15) six (6) inches above the ground.  The employees used a homemade plywood box on a PIV to gain access to the roof of the house.
