29 CFR 1926.502(a)(2): Employers shall provide and install all fall protection systems required by this subpart for an employee, and shall comply with all other pertinent requirements of this subpart before that employee begins the work that necessitates the fall protection.    a) On or about January 13, 2017, at the above jobsite location -  an employee performing electrical operations from top of the freezer was exposed to a fall, of twenty five (25') feet, created by the presence of unprotected sides/edges.  The employee was not provided with a complete personal fall arrest system or guardrail system, thereby exposing the employee to fall hazards of approximately twenty five (25) feet.

29 CFR 1926.503(c): "Retraining." When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer shall retrain each such employee. Circumstances where retraining is required include, but are not limited to, situations where:    a) On or about January 13, 2017, at the above jobsite location -  an employee conducting electrical operations were not provided training on how to minimize falls associated in the workplace, thereby exposing the employee to fall hazards of approximately twenty five (25) feet.
