29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees: Rotary Slitter: On, or about, January 17, 2017, the John Robinson & Company slitter was routinely turned on and off using a 220 volt, three phase disconnect box located on the side of the machine. Recognized and generally accepted electrical standards require that control circuits for industrial machines do not engage more than 120 volts, single phase when operated and that a stop control be provided at the operators normal work position.
