29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment was not physically located and operated in such a manner as to isolate the machine or equipment from the energy source: (a) Facility - 33 Fayette Street, Belle Vernon, PA: On or about January 9, 2017, an employees hand was caught in a belt and pulley located in the Soft-Serve Freezer ice cream machine, due to not isolating all energy sources while trying to clean a leak in the machine. (b) Facility - 33 Fayette Street, Belle Vernon, PA: On or about January 18, 2017, employees are not isolating all energy sources from the Soft-Serve Freezer ice cream machine by disconnecting the main power supply while conducting cleaning activities.
