29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration; Such activities were not performed only by experienced and trained employees selected for such work by the competent person:    On or about January 18, 2017, at the above addressed jobsite, employees were observed operating a suspension scaffold/Boatswains Chair which had been erected, moved, dismantled and altered, while not under the supervision and direction of a Competent Person (capable of identifying the known hazards of the system), and for which the scaffold system/Boatswain Chair was observed to have several safety deficiencies, therefore exposing the employees to a fall hazard.    NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1926.451(d)(5)(iii):     Suspension scaffold support devices such as cornice hooks, roof hooks, roof irons, parapet clamps, or similar devices were not secured against movement by tiebacks installed at right angles to the face of the building or structure, or by opposing angle tiebacks secured to a structurally sound point of anchorage     On or about January 18, 2017, at the above addressed jobsite, employees were observed operating a suspension scaffold/Boatswain s Chair, which was suspended from a parapet clamp.  The single tieback employees used for the parapet clamp was not installed perpendicular to the face of the building.  Further the tieback was improperly attached to the front of the parapet, which created an unsound anchorage point for the tieback.  As a result, employees were exposed to a fall hazard.

29 CFR 1926.502(d)(20): The employer did not provide for prompt rescue of employees in the event of a fall or did not assure that employees were able to rescue themselves:    On or about January 18, 2017, at the above addressed jobsite, employees were using Personal Fall Arrest Systems while working on a rooftop/stairwell tower of a high rise building, for which the employer did not provide for prompt rescue of employees in the event of a suspended-fall, or did not assure that employees were able to rescue themselves (e.g. suspension trauma relief straps, signaling device, self-rescue devises, onsite equipment, technical rescue devices, etc.), therefore exposing the employees to post-fall injuries (suspension trauma/orthostatic shock) resulting from a fall.     Note: Suspension trauma/orthostatic shock can be fatal in as little as 10 minutes, and is typically fatal in 15 to 40 minutes.    NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    On or about January 18, 2017, at the above addressed jobsite, employees accessing the roof of a stairwell tower from an aerial lift; wherein the stair tower roof contained only a 18 inch parapet wall, and wherein the employees were dismounting the aerial lift, and then disconnecting their lanyards from the aerial life anchor, and then in-turn re-connecting their lanyards to an non-complaint anchor point (thought the parapet wall scupper drain), therefore exposing the employees to a fall hazard.    FIRST REPEAT: MCW ACQUISITION, LLC., and DBA METRO CAULKING & WATERPROOFING, LLC. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR. 1926.501(b)(1) and 1926.501(b)(10), or its equivalent standard, which was contained in OSHA inspection number 977396, citation number 01, Item number 001, and was affirmed as a final order on 09/04/2014, with respect to a workplace located at 133 Pompano Beach Blvd, Pompano Beach, FL 33062, and inspection number 957954, citation number 01, Item number 001, and was affirmed as a final order on 08/12/2014, with respect to a workplace located at 133 Pompano Beach Blvd, Pompano Beach, FL 33062.      NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
