29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: a) Scissor Lift, Adjacent to Roof Canopy - On or about January 19, 2017, an employee working in the basket of an scissor lift was exposed to head injuries from falling objects and tools such as, but not limited to, a portable drill and a leaf blower being used by the employee working on the roof canopy.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Roof Canopy - On or about January 19, 2017, the employer did not ensure that employees were protected by safety net systems or personal fall arrest systems while walking/working on the roof with sides that were unprotected by guardrail systems, thus exposing employees to fall hazards up to 20 feet onto dirt and gravel ground.
