29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. a) East side of the building, on or about January 19, 2017, four employees were working from a tubular welded frame scaffold without guardrails and no other means of fall protection, thus exposing them up to 14-feet fall hazard.

29 CFR 1926.452(c)(4): End frames of tubular welded frame scaffolding were not locked together vertically by use of pins or equivalent means in areas where uplift could occur: a) East side of the building, on or about January 19, 2017, a tubular welded frame scaffold that was two sections high was missing pins to secure each section together, exposing employees up to a 14-foot fall hazard.
