29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use    Paint Shop;    On or about 1/19/17, the employer did not have a written respiratory program to cover the use of tight-fitting half face air purifying respirators used by employees who are prepping and painting.  The chemicals used include xylene and isocyanates which cause central nervous system depression and skin/respiratory sensitization.

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace    Paint shop:    On or about 1/19/17, an employee working in the paint shop was provided and required to use tight fitting half face air purifying respirators when painting was not provided a medical evaluation to ensure that using a respirator did not pose a health risk to the employee.

29 CFR 1910.134(f)(1): The employer did not ensure that employee(s) required to use a tight-fitting facepiece respirator passed the appropriate qualitative fit test (QLFT) or quantitative fit test     Paint shop;    On or about 1/19/17, employer did not provide a fit test to an employee required to wear a respirator while spray painting.  Painting is done using materials that contain hexamethylene diisocyanate, which causes respiratory sensitization.
