29 CFR 1910.36(g)(2): An exit access was not at least 28 inches wide at all points: On the Processing Floor, employees working with Flank Steak at Front Pack-off were exposed to fire and asphyxiation hazards in that the exit access between the workstation and the stairway was found to be approximately 19 inches in width.

29 CFR 1910.212(a)(3)(iii): Special hand tools for placing and removing material were not provided to permit easy handling of material without the operator placing a hand in the danger zone: On or about 01/16/17, an employee engaged in meat cutting activities with a Butcher-Boy band saw in the Loin Chine Saw area was exposed to amputation hazards in that special hand tools, including but not limited to hooks, were not in use to keep the employee's fingers from contacting the saw blade.

29 CFR 1910.178(q)(7): Industrial trucks were placed in service after the examination revealed condition(s) adversely affecting the safety of the vehicle. Reported defects were not corrected. In Department #3465, the employer did not remove from service and corrected a defective tire on the Yale forklift (Model-GPL30VXNRSE084 SN-D810V02100P) after examination revealed the deficiency.
