29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    a. On or about January 18, 2017, at the Ryan Homes residential site, employees using a circular saw and nail guns while performing framing activities were not protected with safety glasses or equivalent eye protection.

29 CFR 1926.404(f)(6): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous:   a. On or about January 18, 2017, at the Ryan Homes residential site, the male end of the portable multi-outlet receptacle used to supply power to a circular saw and other equipment was missing the ground prong, thereby exposing employees to an electrical hazard.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502.  Note: There is a presumption that it is feasible and will not create a greater hazard to implement at least one of the above-listed fall protection systems. Accordingly, the employer has the burden of establishing that it is appropriate to implement a fall protection plan which complies with 1926.502(k) for a particular workplace situation, in lieu of implementing any of those systems:    a. On or about January 18, 2017, at the Ryan Homes residential site, employees performing framing activities including the installation of decking on the second level were not protected by a guardrail system, safety net system, or personal fall arrest system, thereby exposing the employees to a fall hazard of 11 feet.

29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws:  a. On or about January 18, 2017, at the Ryan Homes residential site, the female end of a flexible extension cord (red/black) used to supply power to a circular saw was not provided with strain relief causing the outer insulation to be pulled away, thereby exposing employees to an electrical hazard.   b. On or about January 18, 2017, at the Ryan Homes residential site, the male end of a flexible extension cord (orange) used to supply power to a saw and/or other equipment was not provided with strain relief causing the outer insulation to be pulled away, thereby exposing employees to an electrical hazard.
