The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. a) Jobsite - The employer did not establish and implement a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energization, start up or release of stored energy could occur and cause injury, the machine or equipment was isolated form the energy source and rendered inoperative. 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. a) Jobsite: Kitchen - An employee was exposed to entanglement hazards from the mixer shaft of a P6 Automatic Fresh Pasta Machine as the machine's safety cover and safety micro switch were in disrepair. An employee manually preparing food in the tank of the pasta machine suffered severe injuries when his arm got entangled with the machine's mixer shaft that did not automatically shut off when the safety cover was lifted and the safety micro switch activated. Noted on or about 1/18/17.
