29 CFR 1926.501(b)(10): The employer did not ensure that each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system, or on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone: a) Job site, at/near 1400 Smallman Street, Pittsburgh, PA: On or about January 19, 2017, during roofing activities, the employer did not ensure that an employee used appropriate fall protection systems/devices where exposed to a fall hazard of up to approximately 30 feet.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder: a) Job site, at/near 1400 Smallman Street, Pittsburgh, PA: On or about January 19, 2017, during roofing activities, the employer did not ensure that appropriate ladder safety practices were followed in that the ladder in use was extended only approximately 1 1/2 feet above the roof line, was not secured and there was no grasping device installed.
