29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program:  a) Throughout Facility - On or about 1/18/2017, employees were exposed to continuous noise at 771% of the permissible daily exposure of 85 dBA (8 hour time weighted average) or equivalent sound levels of 105 dBA during a 413 minute sampling period on 1/18/2017. The exposure calculation included a zero increment of the 67 minutes not sampled. A noise monitoring program was not provided for employees operating granite crusher machine.   b) Throughout Facility - On or about 1/18/2017, employees were exposed to continuous noise at 109.4% of the permissible daily exposure of 85 dBA (8 hour time weighted average) or equivalent sound levels of 91 dBA during a 406 minute sampling period on 1/18/2017. The exposure calculation included a zero increment of the 74 minutes not sampled. A noise monitoring program was not provided for employees operating cyclone, hopper, shaker, and crusher machines.   c) Throughout Facility - On or about 1/18/2017, employees were exposed to continuous noise at 69.8% of the permissible daily exposure of 85 dBA (8 hour time weighted average) or equivalent sound levels of 87.4 dBA during a 414 minute sampling period on 1/18/2017. The exposure calculation included a zero increment of the 66 minutes not sampled. A noise monitoring program was not provided for employees operating cyclone, hopper, shaker, and grinder machines.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a) Throughout Facility - On or about 1/18/2017, employees were exposed to eye, skin and respiratory tract irritations when the employer did not provide training for employees working with chemicals such as, but not limited to Methyl Methacrylate, Polymethyl Methacrylate (PMMA), Hi-Macs Acrylic Sink and Hi-Macs Joint Adhesive.
