29 CFR 1910.242(a): Hand and portable powered tools or equipment were not kept in safe condition, including tools and equipment which may be furnished by employees: a) Xator Corporation at 2605 E 8th Street - Greeley National Guard Station, Evans, CO 80634: On or before January 19th, 2017, the employer did not ensure that the Milwaukee portable 1-3/4" Heavy-duty Rotary Hammer Drill, Serial #188A101030091, furnished by an employee for use on site, was in safe and proper operating condition. The drill bit seized onto the extension, an in an attempt to disengage the drill bit extension, an employee experienced the amputation of a finger when working with the energized hammer drill. This condition exposed an employee to an amputation hazard.
