29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):      (1) On Jan 17, 2017, at the facility located at 18201 E 23rd St, Independence, MO., employees were exposed to potential struck by injuries in that none of the Powered Industrial Truck Operators had successfully completed the required Training and Evaluation as specified by this standard.

The only logs available were 2016 and 2013.
