29 CFR 1910.178(m)(7): Brakes were not set and/or wheel blocks were not in place to prevent movement of trucks, trailers, or railroad cars while loading and unloading: (a) NDC East Dock Area - On 12/12/2016, wheel blocks did not prevent movement of trucks and/or trailers. Snow and ice combined with an electric chock with a worn and defective stainless steel saw tooth backplate restraint caused the trailer to slide away from loading dock door 120 while loading the trailer using a forklift. The dock leveler lip fell away due to the trailer sliding away from the dock creating an approximate one foot gap between the dock and the trailer. (b) NDC East Dock Area - On 12/17/2016, wheel blocks did not prevent movement of trucks and/or trailers. Snow and ice combined with an electric chock with a worn and defective stainless steel saw tooth backplate restraint caused the trailer to slide away from loading dock door 127 while loading the trailer using a forklift. The dock leveler lip fell away due to the trailer sliding away from the dock which caused a forklift to fall between the dock and the trailer and be suspended by the dock and trailer. (c) NDC East Dock Area - On or about 12/29/2016, wheel blocks did not prevent movement of trucks and/or trailers. Snow and ice combined with an electric chock with a worn and defective stainless steel saw tooth backplate restraint caused the trailer to slide away from loading dock door 120 while loading the trailer using a forklift. The dock leveler lip fell away due to the trailer sliding away from the dock leaving approximately one inch of overlap between the dock plate and the trailer. Or, "IN THE ALTERNATIVE" 29 CFR 1910.178(k)(1): The brakes of highway truck(s) were not set and wheel chocks placed under the rear wheels to prevent truck(s) from rolling: (a) NDC East Dock Area - On 12/12/2016, electric wheel chocks placed under the rear wheels did not prevent truck(s) from rolling. Snow and ice combined with an electric chock with a worn and defective stainless steel saw tooth back plate restraint caused the trailer to slide away from loading dock door 120 while loading the trailer using a forklift. The dock leveler lip fell away due to the trailer sliding away from the dock creating an approximate one foot gap between the dock and the trailer.
