29 CFR 1926.451(a)(1): Scaffold(s) and scaffold(s) component(s) were not capable of supporting, without failure, its own weight and at least four times the maximum intended load applied or transmitted to it. Jobsite: On or about January 20, 2017 an employee at the jobsite in Cranston Rhode Island, was observed installing the roof overhang while standing on the guard rails of the scissor lift. The ER did not ensure the guard rails of the Genie GS-3384 scissor lift were capable of supporting without failure its own weight and at least four times the maximum intended load applied before employees stood on the rails while performing work.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. Jobsite: On or about January 20, 2017 employees were exposed to a fall hazard of approximately 15 feet while working on a steep roof without the use of a guardrail system, a safety net system or a personal fall arrest system.
