On or about 1/25/17 in the Opening Department; the employer allowed employees, and the employees were observed operating the Toyota (Model #8FDU25, Serial # 31230) and the Hyster propane fork trucks without using the seat belts provided as a restraint device; thus exposing employees to serious injuries in the event of a tip-over.
