Listed, labeled, or certified equipment shall be installed and used in accordance with instructions included in the listing, labeling, or certification.  a. On or about January 19, 2017, at the residential Medallion Home site, the duplex receptacle used to supply power to a DeWalt compressor and Bosch radio was not listed or labeled for use outside of an enclosed wall or similar structure, thereby exposing employees to an electrical hazard.

29 CFR 1926.405(a)(2)(ii)(I):  Flexible cords and cables shall be protected from damage.  Sharp corners and projections shall be avoided.  Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage.   a. On or about January 19, 2017, at the residential Medallion Home site, the outer protective covering on the flexible cord connected to the duplex receptacle used to supply power to a DeWalt compressor and Bosch radio was damaged, thereby exposing employees to an electrical hazard.

29 CFR 1926.405(g)(2)(iv):  Strain relief.  Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.   a. On or about January 19, 2017, at the residential Medallion Home site, the flexible cord connected to a duplex receptacle used to supply power to a DeWalt compressor and Bosch radio was not provided with strain relief, thereby exposing employees to an electrical hazard.   b. On or about January 19, 2017, at the residential Medallion Home site, the female end of the flexible extension cord used to supply power to a Rigid circular saw (S/N: CS15144NC19752) was not provided with strain relief which caused damage to the outer insulation of the cord, thereby exposing employees to an electrical hazard.

29 CFR 1926.501(b)(13):  "Residential construction."  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.  Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502.  Note: There is a presumption that it is feasible and will not create a greater hazard to implement at least one of the above-listed fall protection systems.  Accordingly, the employer has the burden of establishing that it is appropriate to implement a fall protection plan which complies with 1926.502(k) for a particular workplace situation, in lieu of implementing any of those systems.   a. On or about January 19, 2017, at the residential Medallion Home site, an employee performing framing activities involving a rafter while on top of an outside wall was not protected by a guardrail system, safety net system, or personal fall arrest system, thereby exposing the employee to a fall hazard of approximately 9 feet (inside) to 15 feet (outside).   b. On or about January 19, 2017, at the residential Medallion Home site, an employee performing framing activities involving the installation of a block while standing on the top plate was not protected by a guardrail system, safety net system, or personal fall arrest system, thereby exposing the employee to an inside fall hazard of approximately 9 feet, 7 inches.

29 CFR 1926.1053(b)(1):  When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grab rail, shall be provided to assist employees in mounting and dismounting the ladder.  In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.   a. On or about January 19, 2017, at the residential Medallion Home site, the side rails on a 10 step aluminum Werner multipurpose ladder extended only 10.5 inches above the top plate that the employee was accessing to perform framing activities, thereby exposing the employee to a fall hazard of approximately 9 feet.
