29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.    On or about January 19, 2017 and times prior thereto, employees of T/A Travel Centers of America, located at or near 3205 Valentine Road, North Little Rock, Arkansas, the flexible power cord of the oil crusher was not equipped with a strain relief device where the cord entered the back of the machine.  This condition exposed employees to the hazard of electrical shock.

29 CFR 1910.334(a)(2)(ii): If there is a defect or evidence of damage that might expose an employee to injury, the defective or damaged item shall be removed from service, and no employee may use it until repairs and tests necessary to render the equipment safe have been made.    On or about January, 19, 2017 and times prior thereto, employees of T/A TravelCenters of America, DBA T/A truck Service, located at or near 3205 Valentine Road, North Little Rock, Arkansas, the outer insulation of the electrical cord of the oil fitter crusher was pulled away from the plug.

29 CFR 1910.253(e)(5)(v): Hose showing leaks, burns, worn places, or other defects rendering it unfit for service shall be repaired or replaced.  On or about January, 19, 2017 and times prior thereto, employees of T/A TravelCenters of America, DBA T/A, located at or near 3205 Valentine Road, North Little Rock, Arkansas, the gas hoses of the oxygen/acetylene torch were worn or damaged at the nozzle.
