29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about January, 2017 and there prior, the following machine guarding violations were observed during the inspection: a. The employer did not guard sprockets, pulleys, and the edges of scrap belts of Flexo/Ward presses, which include but not limited to Flexo 423, 433, Ward 453, and 451 exposing employees to caught-in hazards at the ingoing nip points and rotating parts. b. The employer exposed employees to caught-in rotating parts hazards from unguarded belts on the north side of Flexo 423 Press during normal operation.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: On or about December 17, 2016 at approximately 9:45am, an employee amputated his right index finger when inspecting a conveyor belt and sprockets. The amputation was reported on January 10, 2017 at about 3:00pm, which was not within the stipulated 24 hours timeframe.
