29 CFR 1910.28(b)(5)(i): The employer did not ensure each employee on a runway or similar walkway was protected from falling 4 feet (1.2 m) or more to a lower level by a guardrail system: (a) On or about August 24, 2016, and at times prior, Creede Repertory Theater, Inc. did not ensure that each employee on a runway or similar walkway was protected from falling 4 feet or more to a lower level by a guardrail system. In the main theater grid, employees were exposed to fall hazards of up to 21 feet while conducting electrical work on top of three walkways located across the grid. Walkways were not guarded by railings. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.28(c): When an employee is exposed to falling objects, the employer did not ensure that each employee wore head protection that meets the requirements of 29 CFR part 1910, subpart I.: (a) On or about August 24, 2016, and at times prior, Creede Repertory Theater, Inc. did not ensure that employees working on the main theater stage were wearing head protection while work overhead was being performed. Employees working on the stage were exposed to falling object hazards in that items such as, but not limited to, light fixtures weighing up to 22 lbs. were installed, repaired, replaced, and/or maintained 21 feet above the stage. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.28(c)(1): The employer did not erect toeboards, screens, or guardrail systems to prevent objects from falling to a lower level: (a) On or about August 24, 2016, and at times prior, Creede Repertory Theater, Inc. did not ensure that toeboards, screens or guardrail systems were erected to prevent objects from falling to a lower level. 1. Employees working on the stage were exposed to falling object hazards in that electricians were required to install, repair, replace, and/or maintain light fixtures and other electrical equipment while working from three walkways 21 feet above the stage. The walkways did not have toeboards, screens, or guardrail systems to prevent items such as, but not limited to, 22 lb. light fixtures, tools, and equipment from falling down to the stage. 2. Employees working on the stage were exposed to falling object hazards in that the guardrail system installed on the perimeter of the grid did not have toeboards or screens installed to prevent items such as, but not limited to, 22 lb. light fixtures, tools, and equipment from falling 21 feet down to the stage.
