29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    The employer is failing to protect employees from fall hazards. This was most recently documented on January 20, 2017 at Lot #8 Elim Ave, Cleaver, Missouri 65631. The employer is failing to protect employees from fall hazards while employees were observed working approximately 22 feet up on a residential roof placing new roofing tiles onto a house with a roof pitch of 8/12.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     The employer is failing to protect employees from easy access to the working surface. This was most recently documented on January 20, 2017 at Lot #8 Elim Ave, Cleaver, Missouri 65631. Employees were accessing a working surface on a steep pitch roof by way of a portable ladder whose side rails did not extend at least 3 feet above the upper landing surface being accessed.
