29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) On or about January 17, 2017 and times prior; proper lockout/tagout shut down procedures: The employer failed to ensure an employee used proper shut down procedures in that he used the e-stop instead of the electrical lockout during clean-up and changing the router head exposing him to the unexpected start up.
