29 CFR 1910.23(c)(8): The employer did not ensure that the cap (if equipped) and top step of a stepladder were not used as steps:  (a)  On or about 01/18/2017 - 110 Plugs Drive, Decatur, AL, Lineage Logistics Warehouse Roof, an employee was standing on the top step of a 4-foot Werner fiberglass stepladder, model #WW-3430-04, to make repairs to the steam line.
