29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s). On or about December 1, 2016, the employer did not conduct frequent or regular inspection of the job site that would of identified fall hazards. This would include on the roof were no fall protection system was in place and the employee fell greater than 25 to lower level. Unfortunately employee died do to injuries suffered from the fall.
