29 CFR 1926.50(e): Proper equipment for prompt transportation of the injured person to a physician or hospital, or a communication system for contacting necessary ambulance service, shall be provided.    a)  On January 16, 2017 at 22220 Herzog Road in Reedsville, the employer did not provided any first aid and did not provide transportation to emergency medical services for an employee who fell 15.5 ft. to frozen ground from a roof eave edge.  The employee was dazed after hitting the ground, became upset and scared when no longer dazed, and had a bleeding face and left foot fractures.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a)  On or about January 16, 2017 at 22220 Herzog Road in Reedsville, employees were sweeping snow off a roof in preparation of placing a rug on a 3:12 slippery work surface at heights greater than 15 ft. without utilizing conventional fall protection equipment.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.    a)  On or about January 16, 2017 at 22220 Herzog Road in Reedsville, employees were not trained in the recognition and avoidance of unsafe conditions during site preparation work at heights greater than 15 ft., including but not limited to, sweeping snow off a roof surface without utilizing conventional fall protection equipment.

29 CFR 1926.760(a)(1): Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level shall be protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems.    a)  On or about January 16, 2017 at 22220 Herzog Road in Reedsville, employees were dismantling an existing steel roof and installing new steel roof panels with unprotected edges and at greater than 15 feet above surface level without utilizing fall protection equipment.  No fall protection equipment was installed for work along the open side edges of the roof panels and the employees remained on the roof when the eave edge fall protection equipment was moved to the next work area.

The employer shall institute a training program and ensure employee participation in the program.    a)  On or about January 16, 2017 at 22220 Herzog Road in Reedsville, the employees were not trained in the recognition and avoidance of unsafe conditions during steel erection activities, including but not limited to, dismantling and installing steel roof panels in close proximity to open edges at heights greater than 15 ft.
