The operators were observed using the tug vehicles without attaching the service brake air lines to the trailers thereby rendering the trailer brakes inoperative. b.. Yard (Tug 140348, SN 303614): On or about January 24, 2017, employees were exposed to struck-by hazards, in that the operators were allowed to move loads of up to 80,000 lbs. prior to receiving operator training on the use, care and inspection of the tug vehicles.
