Yard (Tug 120862 VIN:4LMBB2113AL021588): On or about January 24, 2017, employees were exposed to struck-by hazards, in that the operators were allowed to move loads of up to 80,000 lbs. prior to receiving operator training on the use, care and inspection of the tug vehicles.  The operators were observed using the tug vehicles without attaching the service brake air lines to the trailers thereby rendering the trailer brakes inoperative.    b..  Yard (Tug 140348, SN 303614): On or about January 24, 2017, employees were exposed to struck-by hazards, in that the operators were allowed to move loads of up to 80,000 lbs. prior to receiving operator training on the use, care and inspection of the tug vehicles.  The operators were observed using the tug vehicles without attaching the service brake air lines to the trailers thereby rendering the trailer brakes inoperative.

Yard: On or about January 24, 2017, employees were exposed to struck-by hazards, in that Yard Tug (#140248/SN 303614 and 120862/VIN 4LMBB2113AL021588) were operated and not tagged out of service. The tugs were observed operating with missing air service lines, and Tug 140348 had inoperable instrument panel warning lights.
