29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:  A.  On or about January 25, 2017, Loading Dock:  The Komatsu 25 propane powered industrial truck was used with 72-inch fork extensions.  The extensions were not marked on the data plate to show the approximate weight of the truck and extension combination at maximum elevation with a load laterally centered.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  A.  On or about January 25, 2017, Shop:  1.  The Grob, Inc. vertical metal-cutting band saw, Type 4V-18, S/N 3760, had a 12-inch portion of the blade which was unguarded, exposing employees to amputation hazards.  2.  The Westinghouse 1/2 HP 1-inch belt sander had an unguarded in-going nip point at the bottom front pulley, exposing employees to amputation hazards.

On or about January 25, 2017, Shop:  Tumb-L-Matic Tumbler #2 was not guarded to prevent employees from accessing the rotating tumbler during operation.  The enclosure door was not interlocked and was kept closed with a removable metal clip.

On or about January 25, 2017, Rear Wall of the Press Area:  An orange flexible extension cord was run through a wall hole to power an electric leaf blower.  The extension cord had been in place for more than 90 days and was not removed from service after the task was completed.  The leaf blower was used in approximately October 2016.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure  A.  On or about January 25, 2017, at the Forming Dies/Tools Rack:  An orange flexible extension cord was used in lieu of the fixed wiring of the building to power a metal-encased desk lamp on a desk at the end of the rack.  The extension cord was affixed to the metal rack with wire ties and black plastic electrical tape.  B.  In approximately October 2016, Rear of Building:  An orange flexible extension cord was used in lieu of the fixed wiring of the building to power an electric leaf blower.  The cord passed through a wall hole which accommodated a clothes dryer vent.
