29 CFR 1926.417(b): Equipment or circuits that were de-energized were not rendered inoperative, and did not have tags attached at all points where such equipment or circuits could be energized: JOBSITE: A light switch, used to power a light fixture was not locked and tagged out prior to an employee splicing electrical wiring for the light fixture. On or about 01/13/2017, an employee sustained severe 3rd-degree burns to parts of left and right hands while splicing electrical wiring for a light fixture that was not locked and tagged out.
