29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  On or about January 25, 2017, during the installation of roofing material for a residential construction project, located at sub lot 210-212 Ashbourne Dr., Westlake, OH, the employer did not ensure employees working from a ladder jack type scaffold were protected from falling by a guardrail, personal fall arrest, or other fall protection system. The employees were working approximately 2 stories above the next lower level (ground).

29 CFR 1926.452(k)(5): Scaffold platforms were bridged on to another:  On or about January 25, 2017, during installation of roofing materials, the employer did not ensure employees were working from a ladder jack scaffold where scaffold platforms were not bridged one to another. The employees were working approximately 19 ft. above the ground.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about January 25, 2017, during the installation of roofing material on a residential home, the employer did not ensure employees were protected from a fall hazard of approximately 19 feet by utilizing a guardrail system, safety new system, and/or other fall protection system or device.
