29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.    At a residential construction site, a masonry employee cutting stone using a Skilsaw Worm Drive SPT 77 Circular Saw on the front side of the building was not provided with eye and face protection exposing the employee to eye and face injuries.

29 CFR 1926.702(i)(1): Masonry saws were not guarded with a semicircular enclosure over the blade:    At a residential construction site, a masonry employee cutting stone using a Skilsaw Worm Drive SPT 77 Circular Saw on the front side of the building, tied the guard with wire to prevent the guard from automatically returning to cover the blade exposing the employee to cuts, lacerations and/or amputation hazards.
