29 CFR 1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met.  a. The employer did not develop a written hazard communication program when employees used items such as but not limited to, degreasers and oils.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and material safety data sheets.  a. The employer did not provide training to employees on the hazards of chemicals, such as but not limited to, degreasers, oils, and lubricants, that would be used while performing service, maintenance, and installation activities.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  a. The employer did not report an in-patient hospitalization of an employee within 24 hours of receiving knowledge of the hospitalization. On or about December 21, 2016, an employee was hospitalized for two days after sustaining a crushing injury to the hand.
