On or about 01/24/2017, at the above addressed worksite, employees were exposed to an 18-foot fall hazard when working from a platform attachment(basket) that was not attached to the forklift which was moved horizontally exposing employees to a fall hazard; and the penalty has been changed from $12,675.00 to $8,872.50.
