29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) Warehouse; On or about January 16 2017, and at times prior, the employer exposed employees to fall hazards of approximately 15 feet, in that an employee utilized an unsecured forklift attachment as an aerial lift while lighting a ceiling-mounted heater without the manufacturer's prior written approval on a Clark propane-powered forklift (Model GC815, SN G127-1294-7228K0).
