(a) 400 ton hydraulic press robotic cell: The employer did not insure guarding was properly implemented on cell safety system exposing employee(s) working on or around robotic press cell to point of operation, ingoing nip point, rotating parts, struck by, and/or crushing hazards that were created when employee entered safety system enclosure to perform an adjustment on production process equipment. The employee entered the robotic cell without removing the enclosure gate key interlock. In the alternative... 1910.147(a)(2)(ii)(A): Lockout procedures were not implemented when servicing and/or maintenance was taking place during normal production operations when an employee was required to bypass a guard or other safety device. (a) 400 ton hydraulic press robotic cell: The employer did not insure Robotic press cell was deenergized and locked out when employee entered the safety system enclosure to perform an adjustment and servicing on production process equipment. This exposed employee(s) working on or around robotic press cell to ingoing nip point, rotating parts, struck by, and/or crushing hazards.

(a) Press number three (3) straight side punch press: Employee(s) performing hands in die part removal were exposed to a pinch point hazard created by automated coil steel in-feed process and the switch activated back stop point at the end of the die fixture. This process subjected the employee to an amputation injury.

29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end projected more than one half the diameter of the shaft. (a) Press Number three (3) straight side punch press: Employees working around the out feed tail end of the conveyor associated with removal of scrap metal produced from punch press operations were exposed to an unguarded shaft end projecting more than one half the diameter of the shaft diameter, and did not present a smooth edge protected by a nonrotating cap or sleeve. This exposed employees to a caught-on hazard and a cut and laceration hazard.
