29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6: (a) On or about January 25, 2017, and at times prior, the employees were exposed to struck-by hazards from a cylinder of compressed carbon dioxide that was not "properly supported to prevent it from being knocked over," per section 3.4.4 of the Compressed Gas Association Pamphlet P-1-1965. The tank was in use and next to the walkway between the kitchen and the storage area. It was not attached with a chain, strap or other mechanism to a the wall or other fixed object. 29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: (a) On or about January 25, 2017, and at times prior, the employer did not ensure that employees were protected from electrical hazards in that the electrical panels on the west and north walls in the north end of the building were blocked from access by pallets and a roller tray that were stored in front of them. 29 CFR 1910.305(e)(2): Switches, circuit breakers and switchboards installed in wet locations were not enclosed in weatherproof enclosures: (a) On or about January 25, 2017 and at times prior, employees were exposed to electrical hazards from the circuit panel that powers the coffee makers, the ice cream machine, and other equipment, which is installed close to the floor that is wet mopped daily, and under water lines that fill the coffee makers. The cabinet housing the panel is NEMA rated Type 1 enclosure which provides protection to the equipment inside from dirt. The location of the panel is likely to experience dripping and light splashing, for which the NEMA Type 2 enclosure is designed. The panel has mounting holes in the back that are open which can let water and other contaminants in. One of the grommets on the east face of the cabinet near the floor does not completely cover the hole it is set in and water and other contaminants can enter the panel in this location as well. 29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: (a) On or about January 25, 2017, and at times prior, employees were exposed to shock, burns, and fire hazards in that the insulation of the power cord to the Bloomfield EBC Integrity coffee maker was separated from the plug causing strain to be placed on the live conductors. 29 CFR 1910.36(g)(2): Exit access(es) were not at least 28 inches (71.1 cm) wide at all points: (a) On or about January 25, 2017, and at times prior, employees were exposed to fire, smoke inhallation and other hazards that require swift exit from the building because the path to the exit was not maintained at 28 inches at the location where the breakfast microwave cart is stored.
