Employees were exposed to contact with parts of fixed electric equipment or circuits which had been deenergized, but not locked out or tagged or both in accordance with the requirements of this paragraph:      a) 401 North Greenbush Road, Resselaer - On January 25, 2017, and at times prior, employer de-energized circuit breakers to replace electric lights on the back side of the building and on poles in the parking lot but did not lock or tag out the circuit breakers.

Note: The written procedures may be in the form of a copy of paragraph (b) of this section:    a) 401 North Greenbush Road, Rensselaer - On January 25, 2017, and at times prior, the employer did not have a written lockout and tagout procedure available at the site or at the company office.
