29 CFR 1910.119(d)(3)(ii): The employer shall document that equipment complies with recognized and generally accepted good engineering practices. On or about January 30, 2017, the employer did not document that its equipment complies with recognized and generally accepted good engineering practice according to the ASHRAE-15 and IIAR-2 ANSI Standards or equivalent. Examples include, but are not limited to: a. West Engine Room - the West double doors located at Electric Room #2 were not tight fitting, permitting passage of escaping ammonia to other work areas (Electric Room No. 2). b. West Engine Room - Inside Electric Room No. 2 there was no visual and audible ammonia alarm. Also the meaning of each alarm shall be marked by signs near the annunciators.
