On or about December 2, 2016, employees were required to work on mobile tube and coupler scaffolds to perform painting activities in the digester and the employer did not provide frequent and regular inspections of the jobsite by a competent person to recognize hazards at the site. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.451(c)(3): Supported scaffold poles, legs, post, frames, and/or uprights were not plumb and braced to prevent swaying and displacement. On or about December 2, 2016, while performing painting activities in the digester, employees were required to work on mobile tube and coupler scaffolds that had been erected on a surface with a slope of at least 13 degrees. The attachment bracing installed by the scaffold erector/vendor to prevent unintended movement of the scaffold tower was removed to allow the scaffold to reach the work area. This scaffold alteration resulted in reduced stability and employee exposure to falls. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

Such activities were not performed only by experienced and trained employees selected for such work by the competent person. On or about December 2, 2016, while performing painting activities in the digester, employees were required to work on mobile tube and coupler scaffolds that had been erected on a surface with a slope of at least 13 degrees. The attachment bracing installed by the scaffold erector/vendor to prevent unintended movement of the scaffold tower was removed to allow the scaffold to reach the work area. This scaffold alteration was not made under the supervision of a competent person qualified in scaffold alteration, which resulted in reduced stability and employee exposure to falls. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
