29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  a.  On or about January 26, 2017, an employee of Funtime, connecting sections of an amusement park ride, was using a body harness that did not have the attachment point at the center of his back, exposing the employee to a to 120 foot fall hazard.

29 CFR 1926.760(b)(3): Each connector working at heights over 15 and up to 30 feet above a lower level, was not provided with a personal fall arrest system, positioning device system or fall restraint system and wear the equipment necessary to be able to be tied off; or was not provided with other means of protection from fall hazards in accordance with paragraph (a)(1) of this section:  a.  On or about January 26, 2017, an employee of Funtime, connecting sections of an amusement park ride, was not using a fall arrest system, while exposed to 120 foot fall hazard.

29 CFR 1926.1053(a)(19): Where the total length of a climb on a fixed ladder equaled or exceeded 24 feet (7.3 m), the fixed ladder was not equipped with either:  (i) a ladder safety device;  (ii) self-retracting lifelines with rest platforms at intervals:  a.  On or about January 26, 2017, an employee connecting a top section of the tower, was climbing a fixed ladder without the use of a ladder safety device, while being exposed to a 120 foot fall hazard.  b.  On or about January 26, 2017, an employee of Funtime connecting a top section of the tower, was climbing a fixed ladder without the use of a ladder safety device, while being exposed to a 120 foot fall hazard.
