29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices:     The employer did not document that the equipment complies with recognized and generally accepted good engineering practices in the following instances:     (a) The following doors were not tight-fitting to prevent the migration of ammonia into other work areas in the event of a release:  The interior door to the plant from Plant 1 (engine room) and the interior door to the plant from Plant 2 (engine room).  These doors were missing door seals along the bottom of the door.  Employees in the general plant areas would be exposed in the event of a significant ammonia release.  The employer did not document compliance, for these doors, with recognized and generally accepted good engineering practices such as, but not limited to, ANSI/ASHRAE Standard 15 Safety Standard for Refrigeration Systems.     (b) The employer did not document that the Water Chiller 2 in Plant 2, a pressure vessel, complied with applicable recognized and generally accepted good engineering practices such as, but not limited to, the ASME Boiler & Pressure Vessel Code, Section VII, Division 1, Rules for Construction of Pressure Vessels and IIAR Bulletin 109, Guidelines for IIAR Minimum Safety Criteria for a Safe Ammonia Refrigeration System, in that the vessel did not have a nameplate which identified the vessel with a National Board number and had the maximum allowable pressure and temperature stamped on it.
