29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) Production Area- On or about January 21, 2017, employees were exposed to machine hazards associated with in-going nip points, during servicing, maintenance and cleaning of grease from a pulley on the 105 KBA Printing Press. The employer failed to insure that the procedure for the press was utilized.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a) Production Area- On or about January 21, 2017, employees were exposed to machine hazards associated with in-going nip points, during servicing, maintenance and cleaning of grease from a pulley on the 105 KBA Printing Press. The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per 1910.147(d)(2) requirements].  As a result, the remaining energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1904.29(b)(3): Each recordable injury or illness was not entered on the OSHA 300 Log and/or an incident report (OSHA Form 301or equivalent) within seven (7) calendar days  of receiving information that a recordable injury or illness has occurred:    a) On January 30, 2017, the employer failed to log injuries within seven calendar days of the occurrence on the OSHA 300 Log, or equivalent form, during the year 2017.  A work-related recordable injury that had occurred on January 21, 2017, was not entered on an OSHA 300 Log as of January 30, 2017.    No abatement certification or documentation is required for this item.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    a) Production Area- On or about January 21, 2017, the employer failed to conduct periodic inspections of each energy control procedure at least annually to verify the procedures were adequate and were being applied properly.  This would include an inspection of the machine specific procedures for the 105 KBA Printing Press.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
