29 CFR 1910.23(e)(1): Standard railings did not consist of a top rail, intermediate rails and posts, and/or did not have a vertical height of 42 inches nominal from the upper surface of the top rail to the floor, platform or runway:  (a) Elko District Fire Office, Warehouse, South Stairs: Employees were exposed to a fall hazard of up to ten feet to the floor below while using a stairway on the dock with fourteen risers that is missing the standard railing with an approximate ten-inch gap between the steps and wire cage.  (b) Elko District Fire Office, Warehouse, North Stairs: Employees were exposed to a fall hazard of up to ten feet to the floor below while using a stairway on the dock with fourteen risers that is missing the standard railing with an approximate nine-inch gap between the steps and wire cage.
