29 CFR 1910.22(a)(1): The employer failed to provide a clean and sanitary condition for all places of employment, passageways, storerooms, service rooms, and walking-working surfaces. a) On or about January 25, 2017, the employer failed to keep working areas clean and free from accumulation of wood dust. Employees were exposed to serious fire and explosion hazards. Kst Test for explosibility of sampled wood dust measured 8.75 bar*meter/second, with a pressure ratio of 5.99 and 10% moisture content. The material is considered explosive. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: (a) On or about January 25, 2017, the employer failed to ensure that each outlet box had a cover, faceplate, or fixture canopy. Employees were exposed to serious fire and electrical shock hazards in the following instances: 1) An outlet box secured to the support pole located next to the wood-burning furnace lacked a cover or faceplate. This outlet was used to power portable fans. 2) An outlet box located under the fire extinguisher near the truss assembly area lacked a cover or faceplate. This outlet was used to power a battery charger for handheld power tools. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

Snap switches mounted in boxes shall have faceplates installed so as to completely cover the opening and seat against the finished surface. (a) On or about January 25, 2017, the employer failed to ensure that each snap switch mounted in a box has a faceplate installed so as to completely cover the opening and seat against the finished surface. A snap switch located between the facility door and truss assembly area lacked a faceplate. Employees were exposed to serious fire and electrical shock hazards. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
